COMPANY CLAIM CONTACT INFORMATION

• Insurance ID card and vehicle registration
• List of emergency contacts
• A medical card listing any allergies to medication or a condition that requires special attention
• Pen or pencils / Paper or notepad
• Flashlight / First- aid kit
• Blanket
• Jumper cables
• Paper towels
• Disposable camera

• Step 1: CALL 911 - If someone has a life threatening injury. Get needed medical attention if the injury is not life threatening. If vehicles are moveable, move to a safe place on the shoulder of the road.
• Step 2: CALL THE POLICE - Call the police directly. A police report is an important piece of information in resolving your claim.
• Step 3: GATHER INFORMATION - Exchange license plate numbers, contact information and auto insurance information with the other parties involved. Make sure to get telephone numbers.
  • Get names of witnesses (if available). The police can be a back up source of information on other parties involved and witnesses
• Step 4: CONTACT THE CLAIM NUMBER ON THE BACK OF YOUR INSURANCE CARD - As soon as possible, to report your claim.
